Shingrix was FDA-approved last fall; the CDC recommends that people age 50 and older get two doses between two and six months apart. The CDC now recommends Shingrix over the Zostavax, an older shingles vaccine that’s been approved for people age 60 and older since 2006.

The shingles vaccine can be an effective way to help prevent shingles. Like with the chickenpox vaccine, getting it doesn’t guarantee you won’t get shingles. But it will most likely reduce the length of time and severity of the disease if you do get it. The risk of shingles is increased in rheumatic diseases. Therefore, some experts encourage the administration of Herpes zoster vaccine to patients under low-dose immunosuppressive therapy. However, it should be noted that the cumulative effects of more than one low-dose immunosuppressive drug could pose a contraindication for live vaccines. The risk of flare caused by infection is often greater than the risks caused by vaccination. For example, in the 2009/10 H1N1 pandemic, AS03-H1N1 vaccine was administered to 90 patients with juvenile idiopathic arthritis. In 59% of the patients receiving methotrexate and in 24% of those receiving etanercept.

There’s no question that people who have immune-mediated inflammatory diseases, such as different kinds of arthritis, have a higher risk of coming down with shingles than otherwise healthy adults, says Elizabeth Kirchner, CNP, of the Cleveland Clinic’s rheumatologic and immunologic diseases department. But if you have an autoimmune disease such as rheumatoid arthritis, the jury’s still out on whether the new shingles vaccine is safe for you.

  7. I have been on Plaquenil and Prednisone since 2002 for SLE. Also have been on Valtrex much of that time for HSV-I oral herpes, fever blisters. I can go a few months without the Valtrex and without blisters, but if I have a lupus flare and start getting regular fevers I have to take the Valtrex to keep the blisters away.

Continued. The good news is that there's a vaccine for shingles. Because the vaccine uses a live virus, people can't get the vaccine while taking anti-TNF drugs, which suppress the immune system. The shingles vaccine, however, contains minuscule amounts of the live shingles virus. In people with normal, healthy immune systems, that’s no problem. They won’t get sick because their bodies produce more than enough antibodies to kill off the introduced virus, and then stick around for years on end, just in case.
